Kerala is once again on high alert after a Nipah virus (NiV) infection was confirmed in a 43-year-old man from Ramanattukara in Kozhikode district, with the patient currently on ventilator support at Kozhikode Medical College Hospital. This latest case, confirmed in preliminary tests with confirmatory results from the National Institute of Virology (NIV), Pune, still awaited, has triggered immediate and extensive contact tracing efforts by state health authorities. Health Minister K. Muraleedharan has advised the public to remain vigilant and cooperate with containment measures, emphasizing the critical need to avoid contact with bats, which are the natural hosts of the deadly virus. The recurring nature of Nipah outbreaks in Kerala, including significant episodes in 2018, 2019, 2021, and 2023, underscores the persistent public health challenge posed by this zoonotic pathogen. With a high fatality rate ranging from 40% to 75% and no specific vaccine or licensed treatment available, rapid response, rigorous contact tracing, and isolation protocols have become the state's primary defense mechanisms. The current patient is suspected to have contracted the infection while cleaning a godown containing bat droppings, highlighting the ongoing risk of spillover from fruit bats. As 77 contacts, including 58 healthcare workers, have been identified and placed under observation or quarantine, the focus remains on preventing secondary transmission. The Indian Council of Medical Research (ICMR) is intensifying efforts to develop indigenous monoclonal antibodies and rapid detection kits, signalling a long-term strategic shift towards enhanced preparedness. This latest incident reinforces the urgent need for continuous surveillance, robust healthcare infrastructure, and public awareness campaigns, particularly during the monsoon season when bat activity, and thus the risk of viral transmission, tends to be higher.
