In a groundbreaking medical first, a genetically modified pig kidney kept a US patient, Tim Andrews, alive and off dialysis for an unprecedented 271 days, or about nine months. This remarkable achievement, detailed in a new paper published in The Lancet on September 3, 2026, not only sets a record for the longest survival of a living human with a pig kidney but also marks the first successful transition from a porcine organ back to a human kidney transplant. Andrews, 66, who received the pig kidney in January 2025 at Mass General Brigham, eventually received a human donor kidney in January 2026, showcasing xenotransplantation potential as a life-saving 'bridge' for patients awaiting human organs. This triumph shines a bright light on xenotransplantation, the process of transplanting organs from one species to another, as a vital solution to the critical global organ shortage, where thousands die waiting for transplants. The pig kidney, sourced from a Yucatan miniature pig named Wilma, underwent 69 specific genetic modifications to prevent immune rejection and reduce infection risks, a complex process made possible by technologies like CRISPR gene editing. Previous attempts at pig-to-human transplants, including the world's first pig kidney into a living person in 2024, had significantly shorter survival rates, highlighting the scale of this new breakthrough by the Harvard Medical School and Mass General Brigham team. Looking ahead, this success is expected to accelerate clinical trials and regulatory approvals for xenotransplantation. US biotech firm eGenesis, which developed the modified EGEN-2784 pig kidney, has already received FDA clearance for a Phase 1/2/3 clinical trial, named RESTORE, aiming to enroll 33 kidney failure patients aged 50-70. While challenges like managing long-term immune rejection and potential physiological incompatibilities remain, Andrews' journey provides immense hope that animal organs could soon offer a temporary lifeline, or even a permanent solution, dramatically expanding the pool of available organs worldwide.
